When we have confirmed an appointment, I will email you a set of required forms. These need to be completed and returned prior to our first meeting. You can either fax them back to me at 866.454.1261 or scan and email the encrypted file to info@drspencergriffith.com. You can then text me your name and the password to 503.781.9085.
